Over time, it’s natural for breasts to change. Factors like pregnancy, weight fluctuations, genetics, and aging can all lead to sagging, loss of volume, or changes in shape. At my practice, two of the most common procedures women ask about are breast lift (mastopexy) and breast augmentation with implants—and often, the right solution may be a combination of the two.
But which option is best: a lift alone, implants alone, or a lift with implants? Let’s explore the benefits, candidacy, recovery, and surgical options so you can make an informed decision about what feels right for you.
Understanding the Procedures
Breast Lift (Mastopexy)
A breast lift focuses on reshaping and lifting the natural breast tissue. It doesn’t add volume with implants—it simply repositions the breasts higher on the chest and removes excess skin to create a firmer, more youthful contour.
There are several types of breast lifts, each designed for different levels of sagging:
- Donut Lift (Periareolar Lift): A circular incision around the areola. Best for mild sagging and minor reshaping.
- Vertical Lift (Lollipop Lift): An incision around the areola with a vertical line down the breast. Provides more significant lift and reshaping.
- Full Lift (Anchor Lift): Incisions around the areola, vertically down, and along the breast crease. Best for advanced sagging and major reshaping.
- Extended Full Lift: Similar to a traditional full breast lift using the anchor incision, but extended further along the breast crease to remove excess tissue and excise the “bra roll,” improving contour in patients with excess lateral breast or side-chest fullness.
Breast Augmentation (Implants)
Breast augmentation is a surgical procedure designed to enhance breast size and shape using implants, typically silicone or saline. Implants come in different profiles—low, moderate, and high—each determining how far the breast projects from the chest. Low-profile implants provide a subtle, natural look with wider base coverage, while moderate-profile implants balance projection with width, and high-profile implants offer more projection with a narrower base for a fuller appearance. Ideal candidates for implants alone are women who desire larger or fuller breasts but already have good skin elasticity, minimal sagging, and do not require a breast lift for reshaping.
Breast Lift With Implants
For many women, combining a lift with implants provides the best of both worlds. A lift repositions and reshapes the breast tissue, while the implant restores or enhances volume. At my practice, we commonly combine implants with a donut or vertical breast lift. These allow for both improved position and added fullness.

- Provides both lift and upper pole fullness (roundness at the top of the breast).
- Ideal for women who have lost both position and volume due to pregnancy, breastfeeding, or weight loss.
Candidacy: Which Is Right for You?
When I meet with patients, I help them determine the best approach based on a few key factors:
- Degree of Sagging (Ptosis): Mild sagging may be corrected with a Donut lift, while significant sagging requires a Vertical or Full lift.
- Desired Volume: If you’re happy with your natural size, a lift alone may be enough. If you want more fullness, implants can be added.
- Skin Quality: Elasticity matters—patients with poor skin tone may benefit more from a lift.
- Goals for Shape: Some women want natural results, while others prefer a rounder, fuller look that only implants can achieve.
Decision Framework & Sizing Considerations
When choosing between a lift alone and a lift with implants, here’s the framework I guide my patients through:
- Breast Position: If the nipples sit below the breast crease, a lift is often recommended to restore a more youthful placement.
- Breast Volume: If you are happy with your breast position but want more fullness or size, implants alone may be the best option.
- Skin Elasticity: Patients with loose or stretched skin typically benefit from a lift, sometimes combined with implants for volume.
- Overall Goals: If both size increase and correction of sagging are desired, a lift with implants is usually the most effective approach.
- Long-Term Results: Considering how breasts will age over time helps determine whether a lift, implants, or both will provide the most lasting and natural outcome.

- Full healing and Normal Lifting: 4–6 weeks.
- Scarring: Fades over time, varies by lift type.
- Breast Implants Alone
- Downtime: 5 days for light activities.
- Full healing and Normal Lifting: 4–6 weeks.
- Scarring: Minimal, usually hidden in natural creases.
- Breast Lift With Implants
- Downtime: 1–2 weeks for daily activities.
- Full healing and Normal Lifting: 4–6 weeks.
- Scarring: Depends on lift type (Donut vs. Vertical).
